Accepting the reality that someone in the family is ailing and experiencing visible decline is not something that family members cannot readily accept. Sometimes, families are not even prepared for this crisis. During this time, families must be properly guided and supported to help them make the right choices for the welfare of their loved ones.
Helping families cope is a process that medical social workers at Personalized Home Health Care are trained to respond to. Using a network of resources, they counsel families while continuously advocating for holistic and effective patient care.
Our MSW staff help patients and family members understand and cope with problems caused by illness/injury through:
- Assessment of emotional and social factors related to patient and family
- Evaluate the home and family environment
- Counseling and education to maximize strengths and coping skills
- Guide with problem-solving interventions
- Assistance in financial matters
- Coordination of community resources
